- Physical form with doctors stamp
- Registration payment
- Any outstanding forms (if you are new to our program you must have a copy of your birth certificate)
- Volunteer deposit checks- made payable to The Farmington Rockets- $100 9this check will not be cashed, we hold the check until to fulfill your obligations).
- Cleats cup shorts
- t-shirt (players will receive a practice jersey during practice)

Pre-Athletic Health Screening for the 2010-2011 School Year

The annual spring schedule for the PRE-Athletic Participation Health Screening has been established through the cooperation of staff physicians at Botsford Hospital and Providence Hospital.
Health screenings are required prior to try-outs or practice of any sport. A physical dated after April 15th 2010 is acceptable the following school year.

Cookie Dough & Wow Green Fundraiser

The cookie dough and Wow green fundraiser are both by Monday August 10th- all payment must be turned in with the orders.
Make all checks payable to The Farmington Rockets- pickup for the cookie dough fundraiser is set for Wednesday August 26th at Highmeadow.
