Pre-Athletic Health Screening for the 2010-2011 School Year

The annual spring schedule for the PRE-Athletic Participation Health Screening has been established through the cooperation of staff physicians at Botsford Hospital and Providence Hospital.

Health screenings are required prior to try-outs or practice of any sport. A physical dated after April 15th 2010 is acceptable the following school year.

FOR: STUDENT/ATHLETES 6th thru 12th grade during the 2010/2011 School Year

WHEN: WEDNESDAY, JUNE 9, 2010 – 5:00 to 7:30 p.m.

WHERE: NORTH FARMINGTON HIGH SCHOOL

COST: Price $15 (Cash Only) – First Come/First Served

Please note that a number will not be issued until payment is made.

The following procedures shall be followed prior to the screening date:

1. The Pre-Athletic Participation Health Screening Form will be available the night of the exams. It is also available on-line at the District website www.farmington.k12.mi.us. (This is the only form that will be honored)
2. The Pre-Athletic Participation Health Screening Form is to be filled out and signed by both the parent/guardian and student prior to the screening for the next school year.
3. The student must present the Pre-Athletic Participation Health Screening Form properly signed to the athletic administrator at the screening site. If the parent/guardian has not completed their part, the student will be given back the form for completion.
